Responsibilities
Build trusted relationships with whānau and undertake assessments; Co-create and review whānau wellbeing plans; Connect whānau with services and resources; Support whānau to build confidence, capability, and independence; Work collaboratively with internal teams and external agencies; Maintain accurate records using client management systems; Monitor and report on whānau outcomes and progress; Promote cultural safety and uphold Ngāti Hine tikanga and kawa; Participate in community engagement activities and events.
