Conduct structured assessment interviews with parents and care providers; observe individuals in home, school, or community settings; review social history and complete functional assessments; prepare and submit assessment summaries and behavior support plan components; advocate for civil rights by presenting plan information to review committees; document progress, activities, and incidents; train and support staff in implementing Individual Support Plans; assist with daily living activities including socialization, personal care, housekeeping, transportation, and financial management; administer and document medications when assigned; monitor health concerns and communicate with nursing staff; transport individuals safely in the community; participate in safety drills and emergency response.
